The North American Registry of Living Wills™ assures that your written wishes are known. Now all your advance medical directives can be stored electronically and made available, on demand, 24-hours/day to any location in North America.

As a member of the Registry, you will be provided with two cards containing your identification and verification numbers. One card can be given to the person who holds your health care power of attorney; the other should be carried with you at all times. Access to the Registry is via telephone and fax. Document requests are made by calling a special number using a touch-tone phone and following the instructions provided. Instructions are provided in English, Spanish and French. Once the request is verified, the directives are faxed automatically. Access via the Internet will be available soon.

No one wants to consider the possibility of becoming incapacitated. But having advanced medical directives and knowing that they are immediately accessible provides peace of mind and a sense of control over the future.

Membership fees are based upon country of origin. See the registration form for your country for exact fees. For example, in the USA, an initial basic two-year membership in the Registry costs only $39.95 for storage of up to 4 pages of directives. Additional pages may be stored at a two-year cost of $5.00/page. Complete document registration might include a living will, health care proxy/durable power of attorney for health care, and wishes regarding organ donation.

Once registered, access to your directives is free and secure. And, maintaining your membership will never exceed $24.95 for subsequent 2-year periods.
